Z\u00e1kladnou a naj\u010dastej\u0161ie pou\u017e\u00edvanou met\u00f3dou je echokardiografia (ultrazvukov\u00e9 vy\u0161etrenie srdca, ECHO). Vykon\u00e1va sa cez hrudn\u00edk alebo cez pa\u017eer\u00e1k (v ur\u010dit\u00fdch indik\u00e1ci\u00e1ch). Pri pa\u017eer\u00e1kovej (transezofage\u00e1lnej) echokardiografii ide o \u010diasto\u010dne invaz\u00edvnu met\u00f3du, preto\u017ee je potrebn\u00e9 zaistenie \u017eilov\u00e9ho pr\u00edstupu (kanyly) kv\u00f4li podaniu liekov a zavedenie sondy do pa\u017eer\u00e1ka na presnej\u0161ie pos\u00fadenie srdcov\u00fdch \u0161trukt\u00far.<\/p>\n<p data-cya11y-org-font-size=\"16\">Niektor\u00e9 ochorenia vy\u017eaduj\u00fa zhodnotenie EKG a\u00a0v\u00fdkonnosti srdca pri z\u00e1\u0165a\u017ei a\u00a0pos\u00fadenie jeho funk\u010dnej rezervy. Z\u00e1\u0165a\u017e mo\u017eno dosiahnu\u0165 fyziologicky bicyklovan\u00edm (bicyklov\u00fd ergometer) alebo podan\u00edm \u0161peci\u00e1lnych liekov (dobutam\u00edn), ktor\u00e9 p\u00f4sobia na srdce rovnako ako bicyklovanie. T\u00fdmto sp\u00f4sobom je mo\u017en\u00e9 dosiahnu\u0165 za\u0165a\u017eenie srdca aj u\u00a0pacientov, ktor\u00ed z\u00a0ak\u00e9hoko\u013evek d\u00f4vodu nie s\u00fa schopn\u00ed absolvova\u0165 vy\u0161etrenie na bicyklovom ergometri.<\/p>\n<p data-cya11y-org-font-size=\"16\">K\u00a0be\u017ene dostupn\u00fdm neinvaz\u00edvnym met\u00f3dam vykon\u00e1van\u00fdm na na\u0161om pracovisku patria aj holterovsk\u00e9 (24-hodinov\u00e9, 72 hodinov\u00e9, 14- d\u0148ov\u00e9) monitorovanie EKG a\u00a024 hodinov\u00e9 meranie krvn\u00e9ho tlaku.<\/p>\n<p data-cya11y-org-font-size=\"16\"><strong>Transtorak\u00e1lna (cez hrudn\u00edk) Echokardiografia-(ECHO)<\/strong><\/p>\n<p data-cya11y-org-font-size=\"16\">Ide o\u00a0ultrazvukov\u00e9 vy\u0161etrenie srdca. Ultrazvuk je zvuk s\u00a0ve\u013emi vysokou frekvenciou, ktor\u00fd je nepo\u010dute\u013en\u00fd. Ja vysielan\u00fd a\u00a0prij\u00edman\u00fd ultrazvukovou sondou, ktor\u00e1 sa priklad\u00e1 na hrudn\u00edk. Odrazy ultrazvuku zo srdca s\u00fa analyzovan\u00e9 pr\u00edstrojom a\u00a0pretransformovan\u00e9 na pohybuj\u00faci sa obraz srdcov\u00fdch \u0161trukt\u00far. Pri vy\u0161etren\u00ed zobrazujeme jednotliv\u00e9 srdcov\u00e9 oddiely a\u00a0chlopne, ich \u0161trukt\u00faru, ve\u013ekos\u0165 a\u00a0funkciu. Po\u010das vy\u0161etrenia pacient le\u017e\u00ed na vy\u0161etrovacom l\u00f4\u017eku, na \u013eavom boku alebo na chrbte. Na vy\u0161etrovaciu sondu sa aplikuje ultrazvukov\u00fd g\u00e9l, a\u00a0n\u00e1sledne sa sonda prilo\u017e\u00ed\u00a0 na hrudn\u00edk. Okrem mierneho tlaku v\u00a0danom mieste je vy\u0161etrenie bezbolestn\u00e9. Samotn\u00e9 vy\u0161etrenie trv\u00e1 15-30 min. v\u00a0z\u00e1vislosti na komplexnosti echokardiografick\u00e9ho n\u00e1lezu.\u00a0 Ultrazvuk nem\u00e1 \u017eiadne v\u00fdznamn\u00e9 nepriazniv\u00e9 \u00fa\u010dinky na organizmus, a\u00a0preto mo\u017eno toto vy\u0161etrenie opakova\u0165 pod\u013ea potreby. \u017diadna zvl\u00e1\u0161tna pr\u00edprava pred vy\u0161etren\u00edm nie je potrebn\u00e1.<\/p>\n<p data-cya11y-org-font-size=\"16\"><strong>Transezofage\u00e1lna (pa\u017eer\u00e1kov\u00e1) echokardiografia (TEE)<\/strong><\/p>\n<p data-cya11y-org-font-size=\"16\">V\u00a0ur\u010dit\u00fdch situ\u00e1ci\u00e1ch je echokardiografick\u00e9 vy\u0161etrenie cez hrudn\u00edk nedostato\u010dn\u00e9 (hor\u0161ia vy\u0161etrite\u013enos\u0165 resp. vizualiz\u00e1cia alebo s\u00fa vy\u0161etrovan\u00e9 oblasti srdca nepr\u00edstupn\u00e9). Vtedy \u00a0vykon\u00e1vame vy\u0161etrenie pa\u017eer\u00e1kovou echokardiografiou. Ultrazvukov\u00e1 sonda sa nepriklad\u00e1 na hrudn\u00edk, ale je umiestnen\u00e1 na konci tzv. endoskopu (tenk\u00e1 ohybn\u00e1 hadi\u010dka), ktor\u00fd sa zav\u00e1dza do pa\u017eer\u00e1ka. Ide o\u00a0podobn\u00fd pr\u00edstroj ako pri vy\u0161etren\u00ed \u017eal\u00fadka (gastroskopia).<\/p>\n<p data-cya11y-org-font-size=\"16\"><strong>Ako sa vy\u0161etrenie realizuje?\u00a0<\/strong>Na \u00favod je zadn\u00e1 \u010das\u0165 \u00fastnej dutiny\u00a0 a\u00a0hltana znecitliven\u00e1 \u0161peci\u00e1lnym sprejom (Lidocain). V\u00a0niektor\u00fdch pr\u00edpadoch pod\u00e1vame do \u017eily tenkou kanylou lieky proti vracaniu (Degan, Ondansetron) a\u00a0lieky na celkov\u00e9 upokojenie (Midazolam). Pacienta ulo\u017e\u00edme do tzv. stabilizovanej polohy na \u013eavom boku. N\u00e1sledne sa zavedie ohybn\u00e1 vy\u0161etrovacia sonda potret\u00e1 znecitlivuj\u00facim g\u00e9lom (Mesocain) na kore\u0148 jazyka a\u00a0pacienta vyzveme, aby prehltol. V\u00a0tomto okamihu sa sonda zavedie do pa\u017eer\u00e1ka alebo hornej \u010dasti \u017eal\u00fadka a\u00a0potom u\u017e prebieha samotn\u00e9 vy\u0161etrenie, ktor\u00e9 trv\u00e1 cca 15-30 min., pod\u013ea zlo\u017eitosti n\u00e1lezu. Po\u010das cel\u00e9ho vy\u0161etrenia je pacient napojen\u00fd na EKG a\u00a0je pod doh\u013eadom vy\u0161etruj\u00faceho lek\u00e1ra a\u00a0vy\u0161kolenej zdravotnej sestry.<\/p>\n<p data-cya11y-org-font-size=\"16\"><strong>Pr\u00edprava pred vy\u0161etren\u00edm.\u00a0<\/strong>Pred vy\u0161etren\u00edm pacient nesmie 6 hod\u00edn jes\u0165, pi\u0165, nie je vhodn\u00e9 ani u\u017eitie dlhodobej peror\u00e1lnej medik\u00e1cie. Po vy\u0161etren\u00ed sa z\u00a0d\u00f4vodu rizika vd\u00fdchnutia potravy po znecitliven\u00ed hltanu nesmie e\u0161te 2 hodiny jes\u0165 ani pi\u0165. \u010casto s\u00fa pri vy\u0161etren\u00ed pod\u00e1van\u00e9 upokojuj\u00face lieky, preto v\u00a0t\u00fdchto pr\u00edpadoch nemo\u017eno hne\u010f po vy\u0161etren\u00ed \u0161of\u00e9rova\u0165. V\u00a0pr\u00edpade podania upokojuj\u00faceho lieku (dormicum) je nevyhnutnos\u0165ou, aby pacienta doprev\u00e1dzal sprievodca. Taktie\u017e star\u0161\u00ed pacienti by mali by\u0165 sprev\u00e1dzan\u00ed inou osobou.<\/p>\n<p data-cya11y-org-font-size=\"16\"><strong>\u00a0<\/strong><strong>EKG Holter<\/strong><\/p>\n<p data-cya11y-org-font-size=\"16\">EKG Holter\u00a0je prenosn\u00e9, neinvaz\u00edvne zariadenie na 24 a\u017e 72-hodinov\u00e9 (niekedy a\u017e 14-d\u0148ov\u00e9) kontinu\u00e1lne monitorovanie srdcovej aktivity v be\u017enom \u017eivote. Elektr\u00f3dy na hrudn\u00edku sn\u00edmaj\u00fa EKG, \u010do pom\u00e1ha odhali\u0165\u00a0 poruchy rytmu a s\u00favislosti medzi pr\u00edznakmi (b\u00fa\u0161enie srdca, z\u00e1vraty) a \u010dinnos\u0165ou srdca.<\/p>\n<p data-cya11y-org-font-size=\"16\"><strong>K\u013e\u00fa\u010dov\u00e9 inform\u00e1cie o EKG Holteri:<\/strong><\/p>\n<ul>\n<li data-cya11y-org-font-size=\"16\"><strong>Priebeh vy\u0161etrenia:<\/strong>\u00a0Mal\u00fd pr\u00edstroj sa nos\u00ed na p\u00e1se alebo na krku, pri\u010dom elektr\u00f3dy s\u00fa pripevnen\u00e9 na hrudn\u00edku.<\/li>\n<li data-cya11y-org-font-size=\"16\"><strong>Denn\u00edk pacienta:<\/strong>\u00a0Po\u010das merania je potrebn\u00e9 vies\u0165 si denn\u00edk aktiv\u00edt a pocity (boles\u0165, arytmia), \u010do u\u013eah\u010duje lek\u00e1rovi anal\u00fdzu.<\/li>\n<li data-cya11y-org-font-size=\"16\"><strong>Doba monitorovania:<\/strong><\/li>\n<\/ul>\n<p data-cya11y-org-font-size=\"16\">Zvy\u010dajne 24 hod\u00edn, ale modern\u00e9 pr\u00edstroje umo\u017e\u0148uj\u00fa aj dlh\u0161ie monitorovanie (48h \u2013 14 dn\u00ed) pre lep\u0161ie zachytenie ob\u010dasn\u00fdch por\u00fach<\/p>\n<ul>\n<li data-cya11y-org-font-size=\"16\"><strong>V\u00fdznam:<\/strong>\u00a0Pom\u00e1ha diagnostikova\u0165 poruchy rytmu (fibril\u00e1cia predsien\u00ed, flutter), kontrolova\u0165 \u00fa\u010dinnos\u0165 lie\u010dby.<\/li>\n<\/ul>\n<p data-cya11y-org-font-size=\"16\"><strong>\u00a0<\/strong>Vy\u0161etrenie je bezbolestn\u00e9 a bezpe\u010dn\u00e9, poskytuje podrobn\u00e9 inform\u00e1cie, ktor\u00e9 be\u017en\u00e9 kr\u00e1tke EKG v ambulancii nezachyt\u00ed.<\/p>\n<p data-cya11y-org-font-size=\"16\"><strong>Ergometria<\/strong><\/p>\n<p data-cya11y-org-font-size=\"16\">Ergometria (z\u00e1\u0165a\u017eov\u00e9 EKG) je\u00a0neinvaz\u00edvne kardiologick\u00e9 vy\u0161etrenie, ktor\u00e9 monitoruje reakciu srdca, srdcov\u00fa frekvenciu a krvn\u00fd tlak po\u010das postupne zvy\u0161ovanej fyzickej z\u00e1\u0165a\u017ee na bicykli alebo be\u017eiacom p\u00e1se. Sl\u00fa\u017ei na diagnostiku ischemickej choroby srdca, por\u00fach rytmu a pos\u00fadenie kond\u00edcie.
